Care Manager

What you'll do

• Assess eligibility and support needs
• conduct home, hospital, school, and community visits
• provide emotional support and guidance
• develop and update individualized care plans
• connect families with healthcare, community, educational, and financial resources
• coordinate communication among families and service partners
• maintain case notes and documentation
• monitor progress and advocate for access to services.
