What you'll do

• Complete comprehensive patient and family assessments and regularly reevaluate nursing needs
• initiate and revise plans of care with physicians and patients
• provide preventive and rehabilitative nursing procedures and prescribed medications and treatments
• counsel and educate patients and families
• identify discharge-planning needs
• assess and report changes in patient condition
• maintain clinical notes and required documentation
• coordinate care with physicians and other health professionals
• participate in on-call duties
• arrange necessary equipment and supplies
• instruct, supervise, and evaluate home health aides and LPN/LVNs.
