What you'll do

• Take weekend calls from patients, families, and caregivers
• assess symptoms and concerns by phone
• dispatch on-call field nurses when visits are needed and brief them before they go
• contact physicians for orders or updates when conditions change
• provide clear instructions and reassurance when a visit is not needed
• document calls in the EMR
• hand off weekend activity to the clinical team on Monday.
