Registered Nurse (RN) Case Manager – Hospice

The Registered Nurse (RN) Case Manager – Hospice role is field-based in Northfield, MN. Pay is $75,000–$90,000 per year. Moments Hospice focuses on comfort, dignity, and compassionate care for patients and families at the end of life. The RN serves as the clinical lead for an assigned caseload, assessing needs, managing symptoms, and coordinating individualized care. Patient and caregiver education and communication with physicians and the interdisciplinary team support holistic hospice care.

What you'll do

• Complete initial and ongoing comprehensive assessments
• develop, implement, and update individualized plans of care with patients and families
• provide skilled nursing care, pain and symptom management, medication administration, and comfort measures
• educate patients and caregivers
• coordinate care and serve as liaison among patients, families, physicians, and the care team
• participate in interdisciplinary group meetings.
