What you'll do

• Participate in the Case Management Program
• assess patients, develop care plans, coordinate services, monitor progress, and evaluate outcomes
• coordinate nursing, utilization management, and social work services
• maintain patient records
• develop individualized treatment plans
• coordinate discharge planning, referrals, and follow-up
• educate and advocate for patients and families
• participate in interdisciplinary meetings
• follow applicable DoD, DHA, facility, HIPAA, and professional standards
• travel off-site when required.
