Community Health Worker

Job overview

The Community Health Worker role is onsite in Woodland, CA, with field work at participant homes and meal sites throughout Yolo County. Compensation is $26–$30 per hour. The role supports Meals on Wheels Yolo County’s mission to nourish and engage vulnerable older adults and people with mobility challenges. This position provides person-centered support to seniors by assessing needs, managing follow-up, and connecting participants with community resources. It helps participants maintain health, independence, and safe aging in place while strengthening awareness of MOW Yolo programs.

What you'll do

• Conduct person-centered participant assessments
• manage a caseload with regular check-ins and reassessments
• coordinate services and referrals
• build relationships with seniors, caregivers, volunteers, colleagues, and community partners
• participate in outreach
• maintain case records and documentation
• communicate emerging needs
• supervise Community Navigators in shared responsibilities.
