What you'll do

• Supervise and support Certified Nursing Assistants under the Director of Nursing
• collaborate with the interdisciplinary team to assess, plan, implement, and evaluate care plans
• complete baseline and periodic assessments
• prepare for physician rounds
• initiate consultations or referrals as appropriate
• complete documentation, evaluations, and reports
• deliver nursing care within scope and facility policy
• monitor changes in residents’ conditions and notify providers as needed.
