The Home Health Registered Nurse – Sagamore/Cape Area role is an onsite, per-diem position serving the Sagamore/Cape Area in Massachusetts. Compensation is $27 per visit, with an additional $5 per visit for Saturday and Sunday visits. Able Home Care’s patient-centered home health team serves the Cape Area community. The nurse delivers individualized skilled nursing care in patients’ homes, coordinates services, and documents assessments and progress. The role supports continuity and quality of care by communicating changes in patient needs and maintaining records in accordance with Medicare and Mass Health standards.

What you'll do

• Deliver skilled nursing care according to individualized care plans
• initiate preventative and rehabilitative procedures and make referrals
• coordinate services and report changes in patient condition and needs
• maintain patient records and OASIS assessments as appropriate
• prepare clinical and progress notes and submit required paperwork daily
• respond to emails and phone calls.
