It also supports clinical quality and patient-outcome goals through collaboration with the care team and participation in quality activities.

What you'll do

• Recommend therapeutic diets based on patient needs, preferences, and treatment changes in consultation with physicians
• review lab results and educate patients on nutrition interventions
• conduct patient assessments and care planning
• participate in monthly interdisciplinary care conferences and QAPI activities
• collaborate with the Medical Director, clinicians, facility administrator, social worker, nurses, and patient care technicians.
