What you'll do

• Coordinate admissions, assessments, social histories, and documentation
• counsel and support residents and families
• uphold resident rights and confidentiality
• participate in interdisciplinary care planning
• coordinate discharge plans and community referrals
• oversee social services compliance, quality improvement, staff education, and department operations
• support resident advocacy and grievance resolution.
