Mobile Registered Nurse (RN) / Medical Case Manager

Job overview

The Mobile Registered Nurse (RN) / Medical Case Manager role is remote and community-based, with field visits across Beaver and Lawrence Counties, PA, and administrative work completed from home. The schedule is flexible Monday–Friday, with occasional evening hours based on member needs. Starting pay is $29.29–$34.02 per hour. The position is a regular, full-time employee role within a behavioral health managed care organization. The case manager conducts face-to-face assessments, coordinates care and discharge planning, educates members and families, and develops integrated service plans. The role identifies clinical, behavioral, social, and environmental barriers, responds to complex crisis calls using established protocols, and documents assessments and care plans in internal systems.

What you'll do

• Conduct face-to-face member assessments in homes and facilities
• coordinate discharge planning and physician follow-up
• educate members and families
• implement clinical interventions and outreach plans
• triage crisis calls using emergency protocols
• enter assessments, case notes, and care plans
• collaborate with caregivers, providers, clinicians, and social service agencies.
