Care Manager

Job overview

What you'll do

• Complete comprehensive assessments
• develop and update Life Plans with individuals, families, and providers
• conduct required face-to-face visits
• coordinate care and health promotion
• support transitions and post-discharge follow-up
• communicate with individuals and families
• manage referrals and follow-up for community services
• maintain electronic health records in compliance with HIPAA and privacy rules
• assist with benefits and report abuse, neglect, or maltreatment.
