What you'll do

• Complete individualized care plans and care coordination in the electronic medical record
• contact patients to provide support and encourage program compliance
• document communications
• communicate emergent needs to the primary care clinician or designee
• keep the team supervisor informed
• coordinate with internal teams, families, and partner facilities
• follow documentation protocols and provide customer service.
