Working with the SAGE Program Manager and other supervisors, this role oversees long-term case management services and coordinates care across DESC programs. It supports staff practice and clinical quality while promoting recovery and continuity of care for consumers.

What you'll do

• Assist with planning and coordinating long-term case management mental health services
• support and train case management staff
• coordinate transitions and care with DESC programs
• participate in psychiatric consultations, supervision, meetings, training, clinical reviews, and case conferences
• coordinate service schedules
• conduct quarterly chart reviews
• oversee clinical documentation and compliance
• supervise and evaluate staff
• track program work
• maintain cooperative relationships with service organizations
• provide clinical case management for a small caseload
• promote outreach and recovery-based services.
