What you'll do

• Provide general medical care and care coordination
• conduct histories, physical exams, preventive assessments, and evaluations for gaps in care
• address acute and chronic primary care complaints
• document diagnoses and care plans
• order or perform appropriate diagnostic testing
• coordinate post-discharge care
• conduct in-person visits and participate in pop-up clinics
• provide alternating on-call coverage for urgent lab results and pharmacy inquiries
• document encounters in the EMR.
