Community Care Connector

Job overview

The Community Care Connector is an onsite, full-time role based in Memphis, TN, with annual compensation of $45,000–$50,000. The non-clinical community health worker supports SisterReach’s YouMeHIV program and its reproductive and sexual justice mission. The role helps people living with HIV reconnect with and remain engaged in medical care through outreach, individualized navigation, referrals, and follow-up. It also coordinates with clinical and community partners to address barriers and support program goals.

What you'll do

• Conduct multi-channel outreach
• co-develop re-engagement plans
• coordinate warm handoffs and appointments with clinical partners
• accompany participants to appointments when requested
• connect participants with social and community resources
• participate in testing and prevention outreach within training and scope
• maintain service data and reports
• coordinate the YouMeHIV Ambassador Program
• collaborate with staff, clinical partners, and community organizations.
