Case Manager, Allentown Area

What you'll do

• Manage a caseload of 70–80 participants
• complete person-centered assessments
• help members access long-term services and supports and Medical Assistance services
• conduct monthly monitoring calls
• develop and update service plans
• communicate with members and providers
• report incidents and allegations of neglect or abuse within 24 hours
• travel to participant homes.
