Adult Case Manager – Community Mental Health

Job overview

The Adult Case Manager – Community Mental Health role is onsite in El Dorado, KS, with most direct services delivered in community and non-office settings. Compensation is $20 per hour. The role supports the Community Support Program at South Central Mental Health Counseling Center. The case manager partners with adults living with serious mental illness and co-occurring substance use disorders, using the Strengths Model and IDDT practices to support recovery. The work promotes client autonomy, community integration, symptom management, and connection to natural supports and community resources.

What you'll do

• Build supportive relationships with assigned clients
• conduct strengths assessments and develop recovery and crisis plans
• deliver direct services in community settings
• review treatment plans quarterly
• use supportive counseling, solution-focused techniques, and behavioral analysis
• connect clients with community resources
• advocate for clients and make referrals
• collaborate with service providers
• complete timely progress notes.
