The Per Diem Case Manager, Clinical Stabilization Services role is onsite in Roxbury, MA, in a clinical residential treatment setting. The stated pay is $25–$35 per hour. The role supports Dimock’s Clinical Stabilization Services treatment team for people receiving residential substance use treatment. The case manager coordinates care and discharge planning to help clients access services, address barriers, and transition back into the community. Working with clients, clinicians, recovery specialists, medical providers, and community partners, the role supports continuity of care before and after discharge.

What you'll do

• Meet with clients for care coordination and aftercare planning
• assess needs, strengths, and barriers
• develop care plans with clients and the treatment team
• coordinate referrals for housing, employment, healthcare, and financial supports
• coordinate with outside providers
• facilitate psychoeducational groups as assigned
• provide transportation when needed
• coordinate discharge and aftercare
• conduct post-discharge follow-up
• assist with crisis management as needed
• maintain timely documentation.
