What you'll do
- Provide physical therapy services according to the physician’s plan of care
- assess and evaluate therapeutic, rehabilitative, and functional status
- evaluate the home environment for safety and comfort
- assess muscle strength, mobility, and gait with a hospice focus
- direct physical therapy treatments and instruct patients/families
- identify therapy needs and report condition changes
- initiate therapy programs and instruct family/caregivers
- provide consultation to patients, families, and caregivers
- prepare clinical notes and summaries
- participate in discharge planning and interdisciplinary group meetings
- supervise Physical Therapy Assistants (PTAs).
