What you'll do

  • Build relationships with children and families
  • co-facilitate individual, parent, and family therapy sessions
  • provide case management, advocacy, and care coordination
  • connect families with community resources
  • collaborate with schools, DCF, hospitals, and service providers
  • support discharge planning and transitions to ongoing care
  • complete documentation and participate in supervision and team meetings.
