The role supports participant safety and continuity of care through clinical judgment, education, documentation, and handoffs across shifts.

What you'll do

• Respond to after-hours calls and conduct clinical assessments and triage
• determine appropriate interventions, including home visits when needed
• coordinate with on-call providers and facilities
• follow participant care plans and provide nursing treatments
• educate and counsel participants, families, and caregivers
• prioritize and delegate tasks
• notify physicians and teams of changes
• support transitions and handoffs
• participate in team huddles and complete clinical documentation.
