Registered Nurse (RN), Home Care — Day Shift

The Registered Nurse (RN), Home Care role is onsite in Mesa, AZ, with compensation of $52–$54 per hour. The nurse supports patients receiving care in their homes. The role provides skilled nursing care for chronic and acute conditions while promoting patient independence and safety. It conducts in-home assessments, develops care plans with physicians, provides nursing interventions and education, coordinates with home health disciplines, and documents care.

What you'll do

• Conduct comprehensive in-home assessments, including OASIS documentation as applicable
• develop and update individualized care plans with physicians
• administer medications, provide wound care and other skilled nursing interventions
• educate patients and caregivers
• coordinate with physical therapy, social work, and other home health team members
• document visits, assessments, and progress in compliance with regulations.
