What you'll do

• Conduct diagnostic assessments and develop individualized treatment plans
• provide psychotherapy and individual, group, and family counseling
• document sessions in compliance with Medicare and facility requirements
• participate in care plan meetings and interdisciplinary discussions
• support residents and families
• collaborate with psychiatric nurse practitioners and facility staff
• educate facility staff on psychosocial needs and referrals.
