Adult Case Manager

The Adult Case Manager role is onsite in Mesa, AZ, with compensation of $22–$22.50 per hour. What you'll do

• Coordinate behavioral and physical healthcare services
• communicate with clinical team participants and staff
• support transition-to-adulthood referrals and primary care coordination
• provide community-based case management and transportation when needed
• navigate patient rights related to treatment access, appeals, and complaints.
