Private Duty Registered Nurse

The Private Duty Registered Nurse works onsite in patients’ homes in San Francisco, CA. Compensation is $60–$88 per hour. Legacy Concierge provides one-to-one home nursing care with structured mentorship and clinical supervision. The nurse delivers individualized skilled care, monitors patient status, and carries out plan-of-care tasks while communicating changes to the RN Care Ambassador and providers as directed. The role supports continuity, safety, and trust for patients receiving care at home.

What you'll do

• Deliver one-to-one skilled nursing care in the patient’s home
• perform comprehensive assessments and monitor, administer, and document vital signs
• administer medications and prescribed treatments according to the plan of care
• support ADLs and IADLs
• perform ordered G/J-tube feeding and medication administration
• provide wound care, dressing changes, tracheostomy and ventilator care, seizure monitoring, oxygen therapy, and oral suctioning as directed
• complete clinical notes
• report changes and concerns
• maintain confidentiality and follow HIPAA and infection-control requirements.
