What you'll do

• Provide hospice nursing care through assessment, planning, implementation, and evaluation
• perform on-call visits and respond to patient needs during assigned weekend shifts
• manage pain and symptoms
• coordinate with physicians and interdisciplinary providers
• assess eligibility and care needs
• update care plans
• provide palliative interventions, education, and family support
• document care
• administer medications
• coordinate equipment and patient needs
• complete death documentation when appropriate.
