Job overview

The Manager of Medical works onsite in Anaheim, CA, with compensation of $75,000–$90,000 per year. The schedule is Monday through Friday, 1:00 p.m.–9:30 p.m. Illumination Health + Home provides recuperative care, emergency shelter, housing, and family services, with a mission focused on compassion, dignity, and safe housing. The manager oversees medical coordination and discharge-care planning for people experiencing homelessness. The role supports follow-up care, service access, client safety, community relationships, and the daily operations and performance of assigned medical coordination staff.

What you'll do

• Oversee discharge care plans and medical follow-up coordination
• monitor complex client cases
• coordinate medication and health education, appointments, and access to care
• maintain service tracking and documentation
• build relationships with service providers
• supervise, train, and evaluate staff
• oversee staffing, metrics, reporting, records, billing reports, and compliance.
