What you'll do

• Identify patients who qualify for care coordination, including those not meeting clinical goals or quality measures and those overdue for visits, labs, or referrals
• arrange follow-up services as appropriate
• support chronic care management for medically complex patients
• identify gaps in care and take appropriate action
• communicate with patients, care teams, and providers to support continuity of care
• identify psychosocial and other barriers and coordinate contact with community resources.
