Mental Health Therapist — Skilled Nursing Facility

The Mental Health Therapist role is an onsite position based in Ellijay, GA, with a Monday–Friday, 40-hour workweek. The hiring range is $70,000–$90,000 per year, with potential additional bonus compensation and a choice of base salary or variable compensation plan. Vitae Health Systems provides integrated clinical services to residents in skilled nursing and long-term care communities. The therapist assesses, diagnoses, and treats emotional and behavioral conditions, supporting residents’ well-being and adjustment to medical and life changes. The role coordinates care with facility staff and the clinical team, provides therapy and crisis support, and maintains compliant clinical documentation.

What you'll do

• Conduct psychosocial assessments
• develop and monitor individualized treatment plans
• provide individual, group, and/or family therapy
• address depression, anxiety, grief, cognitive decline, and adjustment to illness or disability
• collaborate with clinical and facility teams
• participate in care planning
• document care
• provide crisis intervention and behavioral management strategies
• educate staff and families.
