What you'll do
- Build relationships with hospital case managers, discharge planners, and physician groups
- review clinical information and collaborate with the interdisciplinary team to evaluate potential admissions
- educate referral sources about facility capabilities and programs
- coordinate referrals from intake through admission
- track referral trends and maintain CRM documentation
- assist with census growth initiatives
- represent the organization professionally and compassionately.
