Contractor Case Manager

Job overview

The Contractor Case Manager role is based in Greenbelt, MD, with work at offices, hospitals, clients’ homes, and other healthcare settings. Compensation is $25–$40 per hour. The role is part of a multidisciplinary HIV Clinical Care Team serving people living with HIV throughout Maryland’s 23 counties and Baltimore City. The case manager coordinates person-centered services that support treatment adherence, stability, and continued access to care. The role includes assessments, individualized care planning, coordination with treatment teams, client education, and timely clinical documentation.

What you'll do

• Facilitate intake, screening, and assessment
• complete assessments on time
• maintain accurate care plans and records
• co-develop individualized care plans and coordinate services
• liaise with treatment teams
• educate clients about medication side effects and lab results
• address financial and psychosocial barriers
• complete encounter notes within 24 hours
• maintain confidentiality, active Maryland nurse or social worker licensure, and required credentialing.
