The role also develops patient and caregiver education, supports clinical research opportunities, and collaborates on outreach so patients receive coordinated, informed care.

What you'll do

• Coordinate new-patient referrals, visit planning, testing, and clinic orders
• manage prescription refills, drug-toxicity monitoring, and prior authorizations
• coordinate outpatient consultations, therapies, and treatment plans
• advise nursing staff on patient medical concerns
• connect patients with community resources
• develop patient and caregiver education and research materials
• identify potential clinical-trial participants
• collaborate on program outreach.
