What you'll do

• Perform initial and periodic in-home assessments and update care plans
• schedule visits with clients
• assess home safety, cleanliness, and infection control
• conduct supervisory visits and evaluate direct care staff
• oversee nursing task delegation under COMAR 10.27.11 and remain available for clinical consultation
• involve clients and families in care planning
• communicate findings to physicians
• address complaints
• verify medications, orders, equipment, and emergency protocols
• complete progress notes, upload assessments to the EMR, and maintain trackers.
