The position supports Saginaw County Community Mental Health Authority’s Autism Services, primarily serving people ages 0–21. The coordinator develops and monitors person-centered service plans and coordinates supports for consumers with autism spectrum disorder. By working with families, caregivers, behavioral health treatment providers, and other professionals, the role helps ensure services are coordinated, responsive, and directed toward each consumer’s desired outcomes and community integration.

What you'll do

• Coordinate services for consumers with autism spectrum disorder, primarily ages 0–21
• monitor behavioral health treatment interventions and coordinate with providers
• prepare assessments and participate in personal futures planning
• develop and monitor individual service and support plans
• organize planning meetings and document outcomes
• link consumers to residential, clinical, public, and community resources
• coordinate communication among caregivers and professionals
• facilitate conflict resolution
• coordinate in-home skilled nursing, respite care, and in-home behavior services.
