Prison Linkage Program Case Manager

The role supports clients who are incarcerated or recently released as they transition to community life. Through case management, advocacy, and coordination with health and community providers, it works toward improved health outcomes, stability, and successful reentry.

What you'll do

• Conduct needs assessments and manage referrals
• maintain client records, care plans, and activity logs
• provide case management and advocacy through visits at prisons, homes, hospitals, and the office
• connect clients with medical care, social services, benefits, and treatment
• coordinate hospital discharge and follow-up
• manage a caseload of 40–45 HIV-positive clients.
