What you'll do

• Conduct health presentations and educational sessions
• build relationships with facility staff, discharge planners, and care coordinators
• provide primary care, preventive care, chronic disease management, and follow-up visits
• collaborate on outreach strategies and patient growth
• document outreach and clinical encounters in line with practice standards and HIPAA
• provide consultations and assessments during outreach visits as needed
• participate in occasional weekend or evening outreach events as needed.
