Enhanced Care Management (ECM) Care Coordinator

The Enhanced Care Management (ECM) Care Coordinator role is onsite in Santa Rosa, CA, with compensation of $30.00–$35.15 per hour plus a sign-on bonus. Turning Point Community Programs provides integrated mental health services, employment, and housing to support recovery, independence, and self-sufficiency. The coordinator helps managed care plan members pursue their expressed goals while living in the community. Through care coordination, outreach, advocacy, and connections to health and social supports, the role supports members’ access to services and continuity of care.

What you'll do

• Maintain a caseload of managed care plan members and serve as their ECM point of contact or lead care manager
• collaborate with the treatment team and oversee ECM services
• conduct in-person outreach
• accompany members to office visits as needed and within plan guidelines
• provide health promotion and self-management training
• arrange transportation and connect members to social supports
• explain member benefits
• advocate with healthcare professionals
• use motivational interviewing, trauma-informed care, and harm-reduction practices
• coordinate with hospital staff on discharge plans and monitor treatment adherence.
