What you'll do

• Conduct home assessments for Personal Attendant Services and determine eligibility
• identify potential needs for Home Health, Hospice, durable medical equipment, or Meals on Wheels
• assist families with Texas Medicaid applications and renewals through the Your Texas Benefits portal
• build trust with prospective clients and convert inquiries
• develop referral relationships with doctors, discharge planners, and DME providers
• conduct daily home visits.
