Hospice Registered Nurse (RN) Case Manager — Houston

What you'll do

• Conduct initial and ongoing comprehensive assessments
• determine hospice eligibility and levels of care
• develop, implement, document, and revise care plans
• educate patients and families
• determine service scope and frequency
• perform preventive and rehabilitative nursing procedures
• apply infection-control precautions
• direct hospice aides and LPN/LVNs
• maintain clinical and progress notes
• communicate changes to physicians and care team members
• monitor assigned cases for payer compliance
• and perform on-call responsibilities as assigned.
