What you'll do

• Coordinate care for patients with specialized and complex needs
• help arrange specialty appointments and transportation
• follow up on attendance and obtain reports for medical records
• communicate results and recommendations to primary care teams
• connect patients and families with eligible community resources
• monitor hospital admissions, progress, and discharge plans
• conduct assessments and provide case management and self-management education
• document education and patient understanding
• collaborate on care plans and referrals
• maintain confidentiality and HIPAA compliance.
