Case Manager, Senior Care

What you'll do

• Take ownership of care outcomes for assigned participants in collaboration with the primary care provider
• conduct assessments and educate participants and support networks
• develop and implement care plans with the interdisciplinary team and participants
• coordinate complex care and transitions
• review test results, document procedures, and follow up on changes
• coordinate discharges, medication needs, and hospice transitions
• support clinic operations as needed.
