Care Manager

The Care Manager role is onsite serving the Catholic Diocese of Crookston, MN. Compensation is not specified. The role exists to help members navigate benefits and healthcare access, resolve barriers, and coordinate support through appointments and follow-up. It contributes by providing a consistent advocate and coordinating with Meta Care’s pharmacists, nutritionists, social workers, care coordinators, and leadership team.

What you'll do

• Help members understand benefits and available resources
• assist with coverage, provider, referral, prior authorization, and access questions
• advocate through healthcare navigation challenges and follow issues to resolution
• identify in-network providers and services
• build relationships through outreach and ongoing communication
• coordinate appointments when needed
• support members after hospitalizations or major health events
• connect members with transportation, home support, wellness, and community resources
• track needs, follow up on open items, participate in meetings, support education initiatives, and maintain timely confidential documentation.
