What you'll do
- Provide nursing case management

- conduct outreach, HIV education, and testing
- support clients with barriers to care, treatment adherence, and medication adherence
- perform nursing assessments and medical triage
- coordinate appointments and reminders
- maintain program and electronic medical records
- support the PrEP program.
