Community Health Worker

Job overview

The Community Health Worker role is onsite in Baltimore, MD, with compensation of $22–$27 per hour. The position supports WellCentric DC’s outreach and Case Management work connecting members with healthcare and community resources. The role exists to build trusted, culturally responsive relationships and help members overcome barriers to care. Through outreach, education, advocacy, resource navigation, and follow-up, the worker supports member engagement and access to recommended services.

What you'll do

• Conduct member outreach by phone and, as assigned, through home or community visits
• build culturally responsive relationships
• provide health education and advocacy
• collaborate with Case Managers
• help members navigate healthcare and community resources and address barriers such as transportation, food, and housing
• reinforce care plans and refer clinical concerns to licensed professionals
• participate in outreach events
• document interactions and referrals
• protect confidentiality and work within the CHW scope of practice.

Why this role
