Integrated Care Case Manager

Job overview

The Integrated Care Case Manager is an onsite role based in Derry Village, NH, with most work office-based or remote via telehealth and some visits in clients’ homes and community settings. Compensation is $26 per hour. The role supports the Integrated Care Program, which coordinates mental and physical health services for people with comorbid conditions.

What you'll do

• Assess clients and develop care plans
• help clients obtain and maintain medical coverage
• coordinate, link, and monitor internal and external services
• provide community-based counseling, Functional Support Services, Therapeutic Behavioral Services, and crisis intervention
• document services
• and collaborate with treatment teams, natural supports, and external providers.
