Enhanced Care Management Specialist

What you'll do

• Engage and support ECM members
• monitor treatment adherence against care plans
• provide health-promotion and self-management training
• communicate regularly with members, including home visits if necessary
• coordinate with stakeholders and providers
• assist with discharge planning and care transitions
• attend medical appointments
• document interactions and referrals
• conduct psychosocial assessments and collaboratively develop treatment plans
• maintain HIPAA confidentiality and regulatory compliance
• contribute to quality improvement
• use electronic health records.
