Case Manager II — T Building

Job overview

The Case Manager II role is onsite in Jamaica, NY, and is full-time at 35 hours per week; compensation is $50,000 annually. CAMBA provides onsite services at the T Building, a residential development that includes apartments for chronically homeless adults with serious and persistent mental illness and low-income community residents. The case manager supports tenants’ stability and progress toward independent living goals. The role assesses needs, develops plans with tenants, connects them to community resources, monitors progress, and coordinates referrals and advocacy with service organizations.

What you'll do

• Maintain confidential client relationships and tenant files
• review eligibility documentation
• conduct intakes, assessments, and reassessments
• develop and revise independent living plans with tenants
• connect tenants to resources and referrals
• monitor progress and document case notes
• participate in case conferences
• advocate and coordinate with outside organizations
• schedule and escort tenants to appointments
• assist with benefits applications
• conduct home visits
• enter tenant data and progress information.
