The role coordinates with interdisciplinary teams and attending physicians to maintain continuity of care, and includes participation in an on-call rotation with 24/7 availability when assigned.

What you'll do

• Evaluate patients for pain, symptom management, hospice eligibility, and recertification
• counsel patients and families on care options and advance care planning
• order tests, initiate treatment, and conduct face-to-face visits as needed
• serve as attending physician when appropriate
• coordinate care with interdisciplinary teams and attending physicians
• manage medications
• participate in assigned on-call rotation and provide 24/7 availability when assigned
• complete timely clinical documentation.
