What you'll do

• Assess and diagnose behavioral health clients
• develop treatment plans and help clients achieve goals
• evaluate crisis situations and apply appropriate interventions
• participate in integrated healthcare meetings
• identify at-risk patients and help develop prevention plans
• support primary care care-management processes and patient education
• provide assessment, consultation and brief intervention
• teach patients, families and staff care and prevention techniques
• monitor the site's behavioral health program and recommend improvements.

Why this role
